A 3x5 index card signed and inscribed by J.T. Barrett IV, who is the only Ohio State quarterback with a perfect 4-0 record against Michigan. Barrett accumulated more than 25 Ohio State records including most total yards in a season (3,851) and the second most touchdowns in a season (47) as well as most career passing yards (9,434) and completions (769). Additionally, he has several Big Ten Conference records including most career passing touchdowns (104) and total touchdowns (147). He also passed Drew Brees for most career offensive yards, with 12,697.

Barrett was a four year starter at Ohio State, including for most of the 2014 National Championship season before an injury sidelined him against Michigan. He was the first QB at Ohio State to start in four wins against the Wolverines.  Because of his redshirt season in 2013, he earned five pairs of Gold Pants over his career. He was a 2014 Third Team All-American, 2016 Big Ten Player of the Year, and a 3x Big Ten Quarterback of the Year. He signed with the New Orleans Saints in 2018.

He signed the index card boldly in black sharpie and added the inscription, "4-0 vs TTUN".
